Final Journeys is a one-day training workshop that aims to improve the delivery of end-of-life care amongst all hospital staff. The workshop aims to improve end of life care through promoting a culture of awareness and personal empowerment across all disciplines in the hospital. It focusses on enhancing the communications skills of participants to enable them to be more competent delivering end-of-life care and supporting patients who are dying, and their families. Final Journeys is about the person’s final journey through the hospital.

Who is Final Journeys for?

It is for all hospital staff in the HSE and voluntary acute hospitals, regardless of their role.  As end-of-life care is everyone’s business, everyone should attend this workshop. 

It’s estimated since 2010 over 5,000 people have participated in a Final Journeys workshop

The programme has been refreshed and updated and the new materials now feature an individual participant workbook, a comprehensive facilitator guide and accompanying activities and audio-visual materials. The materials include exercises, video clips and skills practice to ensure the workshop is interesting and interactive with a strong emphasis on the application of learning back in the hospital setting.

Delivery 

The workshop is delivered onsite by acute hospital staff trained as Final Journeys facilitators. Each workshop is facilitated by two trained facilitators.

IHF are currently working with the HSE to upskill existing Final Journeys facilitators and train new ones to ensure the continued and sustainable success of the programme throughout Ireland.

Can I become a Final Journeys facilitator? 

If you are working in an acute hospital and are passionate about delivering quality end-of-life care and creating a ‘Hospice Friendly Hospital’ you can request to become a Final Journeys facilitator, providing you meet the selection criteria. Once trained as a facilitated you are able to commit to facilitating a minimum of 2-3 Final Journey workshops per year.
